Beyond the basic semen test

DNA fragmentation

Sperm can look and swim normally while carrying damaged genetic material. DNA fragmentation testing measures the share of sperm with broken or damaged DNA strands. High fragmentation is linked to lower conception rates and miscarriage, and it is worth considering after unexplained infertility or recurrent pregnancy loss.

Oxidative stress

Much DNA damage is driven by oxidative stress, an imbalance between damaging molecules called reactive oxygen species and the antioxidants that neutralise them. Smoking, infection, heat and obesity all raise it. The encouraging part is that it is one of the more modifiable underlying causes.

A normal-looking sample can still carry hidden damage, which is why deeper tests exist.

Hormone panels

Sperm production is driven by hormones, mainly FSH, LH and testosterone. A blood panel can reveal whether low output is a signalling problem from the brain, a problem in the testes themselves, or something else. Hormone testing is especially useful when count is very low or absent.
